Two things that need to go: MC and GMC gaining cover by having a toe in something that only comes up to their knees, and FMC gaining any cover saves/modifiers from things on the ground. It's mind blowing that a FMC can claim the Hard to Hit rule, but then suddenly be down on the ground cowering behind a wall 1/2 an inch tall.
